Lapse
Termination of coverage because of nonpayment within a specified time period.

Length of Stay (LOS)
The total number of days a participant stays in a facility such as a hospital.

Lifetime Maximum
Maximum amount of benefits available to a member during their lifetime. All benefits furnished are subject to this maximum unless stated as unlimited.

Long-Term Care
A continuum of maintenance, custodial, and health services for the chronically ill or disabled, such services may be provided on an inpatient (rehabilitation facility, nursing home, mental hospital) or outpatient basis or at home.

Long Term Care Facility
Usually a state licensed facility which provides skilled nursing services, intermediate care and custodial care.
