Denticare Individual Dental Discount Plan

There is no charge to the individual for using the services of a licensed health insurance agent—the commission is entirely paid by the carrier. The carrier is prohibited from selling the policy any cheaper if an individual does not use an agent.

Dental programs come in two basic forms. Dental insurance which pays a percentage of the cost of services and allows you to receive services from any dentist. Prepaid or capitation plans which have agreements with specific dentists and charges you a set co-payment for services provided by one of the contracted dentists. Please note that the Denticare plan here is NOT insurance.  They do not make payments directly to medical providers.  It is a discount program, and you are obligated to pay for all health care services.  You will receive discounts for medical services at certain health care providers who have contracted with the plan.  The programs and its administrators have no liability for providing or guaranteeing service or the quality of service rendered.

Plans start only on the 1st of the month.   The cutoff date for doing the electronic application and paying monthly is the 15th of the month in the prior month.  For example, an electronic on or before September 15th would be in effect October 1.   You can still get an October 1 effective date after September 15th but you would have to pay the annual premium.

When you enroll in Denticare, treatments you receive from your participating dentist will be provided at discounted fees called co-payments. There is no waiting period for services. For example, if you need a crown you would be able to make an appointment with your participating dentist and get a crown, or dentures, or whatever dental services you need.

To Apply

Download the brochure, application and list of dentists.  Fill out the application and select a dentist.  If you are paying by credit card you can fax the application to 407-386-7053.  If you are paying by check then mail the application with a check made payable to DentiCare. void check and Automatic Monthly Bank Draft to:

Further instructions:
It may be difficult to read all parts of the application on some faxes.  The middle section of the application that is dark is for dependents, name, relationship, date of birth, sex.  The section below regards your payment options.  If you pay by credit card DentiCare will bill your card for the entire year.  If you pay by check, they require Bank Draft or annual payment.  When paying by credit card be sure to note what type of card, Visa, Mastercard, American Express or Discover and the Expiration Date.  You can put this information on a separate piece of paper if necessary.  Be sure to remember to include the monthly fee and enrollment fee if paying by check.  Applications by mail need to be received by the 15th for a 1st of following month effective date, faxed applications must received by the 18th.
